Check a signed form

When a preceptor signs a student’s Clinical Form or Patient Contact Form on their own phone, the PDF gets a code at the bottom. Enter it to see exactly what the preceptor signed. If the PDF doesn’t match, it was changed after signing.

Signed copies are kept for 120 days, then deleted.
